Summary: Fixed quantitative covariates

  Fixed covariates (dummy or quantitative)
with missing values have never been allowed but are ERRORS and
program quits. Standard deviations of fixed covariates were
wrongly computed. Mean and standard deviations of time varying
covariates are still not computed.
